Sign-Up Process

1) Download & read the required release forms

All appropriate pages in our forms pdf* file must be read, understood, signed, sent to Dr. Melody for review and be on file before you are able make your payment and start attending your sessions. For the release section of our forms, you are only required to fill out the Yoga or the Health Coaching Release Form as appropriate for the session you are signing up for.

*Note: Please rename the pdf file for your returned form, replacing “Your Name” with your actual name, and “Date” with the date you finished filling out the form.

2) Fill-out the forms & submit them

We encourage you to take your time when filling out the forms and to provide accurate information.
Once complete, submit your forms to Dr. Melody here:

3) Submit payment, receive session information & your schedule

Once we receive and have reviewed the information provided in your forms, we will contact you via email with a payment link and available dates for your session. Once payment is received, a link for your session and/or other information will be provided.*

*A zoom link is typically provided. If your session requires a phone call or alternate arrangements are required, we will contact you at the email address provided. If you have changes to your preferred time or have to cancel, please contact us right away.
